Hello CMS parents. We are excited for another fun filled year of CMS athletics! As we get closer to the school year, we wanted to get you updated on our pre-sports processes. Please understand that all this will need done before your child can take part in any sports related activity, including tryouts. 

Firstly, please make sure to register on Final Forms for the school year. To do this go to finalforms.com. Click Login, type Claremont into the School district search bar and click the prompt that shows up, as shown in the example below. You will be walked through the rest.

Next, your child will need an updated physical if they don't have one, that is valid through the sports season. Physicals are valid for one year. You may take a photo of your updated physical form and upload it to Finalforms, you may bring a physical copy to Doug Beaupre during ImPACT testing or you may be able to get your doctor's office to fax it to the school. 

I will share the ImPACT testing schedule with you here. Please be sure to make your scheduled time slot the best that you can. This is part of our concussion protocol and you will not be able to compete without having done it. If you cannot make the ImPACT testing week for any reason, please reach out to @Douglas Beaupre for your options on retaking it.
